This day was instituted in Argentina by Pope John Paul II to coincide with the Feast of the Annunciation. John Paul II viewed the day as a positive alternative in favor of life and the propagation of a culture of life to assure respect for human dignity in all situations. El Salvador was the first nation to recognize the 1993 establishment of the Day of the Right to Be Born.

Following suit, Argentina, Chile, Guatemala, and the Republic of Costa Rica established the Day of the Unborn in 1998, the Day of the Conceived and Unborn in 1999, the National Day of the Unborn in 2000, and the National Day of Life Before Birth in 2001. The first nation to observe the Day of the Unborn Child was Nicaragua in 2000, followed by the Dominican Republic in 2001 and Peru in 2002. The Knights of Columbus have embraced International Day of the Unborn Child promotion.
