In a shocking turn of events, officials used a bogus identity to apprehend Messina Denaro on January 16, 2023, when he was seeking treatment at a clinic. He was then moved to the L’Aquila San Salvatore hospital’s inmates’ ward, where he had been receiving treatment as his condition declined. Following recent rumours that he had entered a “irreversible coma,” it was decided to stop feeding him and performing CPR.

Messina Denaro maintained control over the Cosa Nostra’s illegal activities although he was a wanted man, earning him a dreadful reputation. He apparently bragged about having enough victims to “fill a cemetery”. He was regarded as the final “secret-keeper” of the Cosa Nostra, and in addition to directing racketeering, illegal garbage disposal, money laundering, and drug trafficking, he held crucial details and names associated with several high-profile Mafia crimes.

By keeping an eye on Messina Denaro’s close friends and associates, Italian authorities had come close to catching him multiple times. This tactic led to the detention of his sister Patrizia and a number of his colleagues in 2013. His connections to valuable companies were also seized, further isolating him.

Few images of Messina Denaro exist despite the prolonged search for him, so law enforcement had to rely on digital composites to piece together his appearance over time. It took till 2021 for the release of his voice recording.
